School Location

Provide us with the name and location of your school. You need to fill in all the fields under School Location. Depending on which country you select in the School Country list, different fields appear.

For schools located in the United States:

1. From the School Country list, select United States.
2. In the School ZIP or Postal Code box, enter the ZIP code where your school is located.
3. From the School Name list, select your school. If your school does not appear in the list:
   1. Select Other.
   2. In the School Name box, enter the name of your school.
   3. In the School City box, enter the city where your school is located.
   4. From the State list, select the state where your school is located.

For schools located in other countries:

1. From the School Country list, select the country where your school is located. If the country does not appear in the list, select Other at the bottom of the list.
2. If you selected Canada for the school country, from the School Province list, select the province where your school is located.
3. From the School Name list, select your school. If your school does not appear in the list:
   1. Select Other.
   2. In the School Name box, enter the name of your school.
   3. In the School City box, enter the city where your school is located.
